Abstract
Strain Counterstrain (SCS) is one of the treatment modalities available to a practitioner of osteopathic medicine. It is an extremely gentle treatment modality which involves a ninety hold in a position of ease determined by the practitioner. It is proposed that, in this period of time, psycho-emotional mechanisms are at work as well as physiological ones, and it is the aim of this study to investigate these potential mechanisms.
Methods
Twenty three musicians (violinists and viola players) were recruited from the London Philharmonic Orchestra (LPO) to take part in a crossover design study comparing the effects of an SCS treatment and a positive visualization treatment (PVT) on cervical mobility. A reliability study was conducted to investigate the reliability of the cervical range of motion (CROM) device. In the main study, assessments of cervical mobility in all vectors were made before and after each treatment using the CROM device.
Results
Both SCS and PVT exhibited significant effects on cervical ROM in four out of the six vectors (p<0.05). However, the SCS treatment resulted in a generally more significant change in ROM (p<0.001), and the percentage change in ROM exceeded the values calculated for the coefficient of variation in the reliability study.
Conclusion
There are very likely to be psycho-emotional mechanisms at work in SCS which may augment the physiological mechanisms. A methodological finding of this study highlights possible post treatment changes in thoracic kyphosis and forward head posture. These changes have been shown to have significant effects on the reliability of CROM measurement, and should be considered in future studies using the CROM measurement device.
