Abstract
Background: The diaphragm is the main muscle of respiration. In various respiratory conditions such as Chronic Obstructive Pulmonary Disease (COPD), it is postulated in the literature, that its functions could be initially compromised leading to diaphragm fatigue. As part of this pathogenesis, this could then lead eventually to progression of clinical COPD. The diaphragm is also mentioned within the research literature as possibly implicated in spinal stability and therefore dysfunction, could lead to Non-Specific Low Back Pain (NSLBP). Additionally the diaphragm also accommodates with digestion as the esophageal tract broaches the diaphragm. It is suggested that this in turn could lead to regurgitation, which potentially may lead to Gastroesophageal Reflux Disease, (GERD). Objective: This study sought to investigate the attitudes of osteopaths practicing in the UK, with regard to choosing the diaphragm as a target when treating underlying conditions such as COPD, NSLBP and GERD. Design: A questionnaire based quantitative study with qualitative elements. Methods: An electronic questionnaire was sent out to 2623 osteopaths registered with General Osteopathic Council. Demographic information was used to assess the relationship with certain demographic variables. Non-parametric data was tested via the Kruskal-Wallis Test. Results: Following exclusions, a total of 259 responses were analysed. The total sample group did show consensus in the opinion that it is beneficial to treat the diaphragm in patients with COPD, NSLBP and GERD – 80%, 73% and 82% respectively. Discussion: The sub-categories applied from the osteopaths contacted, showed no statistically significant results as to any grouping, trends or general consensus from the osteopathic community under scrutiny, in all but one area: That it is beneficial to treat the diaphragm in COPD, NSLBP and GERD according to the attitudes of the osteopaths. Conclusion: The majority of the osteopaths found it beneficial to treat the dipahragm in patients with COPD, NSLBP nd GERD. However, due to lack of research, theses opinions are mainly experience-based. Further studies investigating the benefits of treating the diaphragm are needed.
