Abstract
Rationale: Idiopathic Parkinson Syndrome(IPS) is the most common neurological Movement disorder. Pathophysiologically, there is a progredient loss of dopaminergic neurons in the Basal ganglia, the aetiology is unknown. IPS leads among other symptomes especially to a severe disability of motor functions. The influence of Osteopathic manipulative treatment in IPS is not very well known. I also investigated whether osteopathic treatment leads to an improvement of the motoric functions of Parkinson patients.

Methods: 23 patients with IPS of Rigor-Akinesia dominance-type were randomised in two groups. All patients were treated twice in an interval of 3 weeks, in a blind cross over design, with an individual osteopathic treatment and a unspecific physical training. There was not a fixed protocol for osteopathic treatment, the techniques were attuned to the individual patient. Motoric tests concerning the gait and pro-supination movements were performed before and after each treatment. 19 patients completed the study.

Results: The speed of gait increased after osteopathic treatment more than 10% (p=0,00 1), it was unchangend in the control group. Speed increase was rather achieved by means of a wider distance than by means of time limits. The time for pro-supination movements was 5% faster in the osteopathic group (p=0,021) and also almost unchanged in the control group. Long-term effects of the osteopathic treatment were not seen.

Conclusions: Osteopathic manipulative treatment with an individual approach can acutely improve the motor skills of patients with IPS, especially the gait. Possible mechanisms of this effect under consideration of the anatomical structures are widely discussed.
