Hintergrund: Osteopathische Palpation stellt hohe
Anforderungen an OsteopathInnen. Neben haptischen Fähigkeiten spielen
Interoception und Intuition eine wichtige Rolle im Kontakt mit PatientInnen, um
Informationen aus dem Körper wahrzunehmen und zu interpretieren. Den
Palpationsprozessen zugrunde liegende neurophysiologischen Aspekte, sind wenig
erforscht.

Ziel: Die Feststellung der Veränderung des Zustandes im autonomen Nervensystem
von OsteopathInnen im Vergleich zu Nicht - OsteopathInnen bei der Zentrierung
im eigenen Körper vor Beginn der Palpation, und während einer craniellen
Palpation.

Methode: Im Rahmen einer
experimentellen Grundlagenforschung mittels einer kontrolliert
randomisierten Studie, wurde durch einmalige Herzratenvariabilitätsmessung bei
15 Osteopath/Innen und 15 Nicht - OsteopathInnen der Zustand des autonomen
Nervensystems während der Zentrierung und craniellen Palpation gemessen und
ausgewertet.



Ergebnisse: Es zeigt sich ein signifikanter
Unterschied zwischen den Gruppen in den HRV- Parametern Total Power (p= 0,036)
und SDNN (p=0,029) während der craniellen Palpation, wobei bei den
OsteopathInnen eine Verschiebung des autonomen Gleichgewichts Richtung
Sympathikus erfolgt. Diese vermehrte sympathische Erregungslage findet sich bei
allen Parametern und Phasen der Messung bei den OsteopathInnen.

Diskussion: Die Einnahme einer entspannten,
zentrierten Haltung der OsteopathInnen, als Voraussetzung für eine adäquate
Palpation, darf nicht in Zusammenhang mit einer Parasympathikusaktivierung im
Behandler gebracht werden. Mechanismen, die in Zusammenhang mit Entspannung
auftreten, bedürfen in der Osteopathieausbildung klarer Definition und Lehre.
Interoception als osteopathische Schlüsselkompetenz sollte nicht außer Acht
gelassen werden.

aspects of osteopathic palpation measured with the heart rate variability
measurement

Background: Osteopathic palpation places high demands on the osteopath.
In addition to haptic skills, interoception and intuition play an important
role in contact with the patient in order to perceive and interpret information
from the body. Little research has been done into the neurophysiological
aspects underlying the palpation processes.

Objective: To determine the change in the state in the autonomic nervous
system of osteopaths compared to non-osteopaths when centring in their own body
before the start of palpation and during cranial palpation.

Method: Within the framework of an experimental basic research using a
controlled randomized study, the condition of the autonomic nervous system
during centring and cranial palpation was measured and evaluated by means of a
single heart rate variability measurement with 15 osteopaths and 15 non -
osteopaths.

Results: There is a significant difference between the groups in the
HRV- parameters Total Power (p = 0.036) and SDNN (p = 0.029) during cranial
palpation, whereby the autonomic balance is shifted towards the sympathetic
nervous system in osteopaths. This increased sympathetic excitement is found in
all parameters and phases of the measurement among osteopaths.

Discussion: Taking a relaxed, centred posture of the osteopath, as a
prerequisite for adequate palpation, must not be associated with
parasympathetic activation in the practitioner. Mechanisms that
relate to relaxation require clear definition and teaching in osteopathy
training. Interoception as a key osteopathic competence should not be
neglected.
