Titel:Einfluss einer osteopathischen Behandlung der Halswirbelsäule bei SchulterschmerzenHintergrund:Neben Rücken- und Nackenschmerzen zählen Schulterschmerzen zu den häufigsten Beschwerden im muskuloskelettalen Bereich.Ziel:Diese Studie dokumentiert den Einfluss einer Behandlung der Halswirbelsäule (HWS) bei Schulterschmerzen und sollte somit Fortschritte beim Evaluieren von Behandlungsstrategien bringen.Methode:Hierbei handelt es sich um eine randomisiert, kontrollierte Studie mit einer Studiengruppe (n=14) und einer Kontrollgruppe (n=14). Die Studiengruppe enthält vier ausgewählte abfolgende Behandlungstechniken an der HWS, die Kontrollgruppe enthält lokale Anwendungen mittels transkutaner elektrischer Nervenstimulation (TENS). Gemessen wird mittels Constant-Murley-Score (CMS), Fragen zur Lebensqualität (SF36) und Schmerzskala (VAS). Es gibt drei Messzeitpunkte mit vierzehntägigen Abständen, zu Messzeitpunkt T1 und T2 erfolgen die Behandlungen parallel zu den Messungen, zu Messzeitpunkt T3 erfolgt die letzte Messung.Ergebnisse:In der Studien- als auch in Kontrollgruppe zeigen sich Verbesserungen innerhalb der Messintervalle. Die VAS zeigt schnellere Verbesserungen in der Studiengruppe. Signifikante Verbesserungen in der Studiengruppe zeigt auch der CMS im Teilbereich Kraft.Diskussion:Die Zeitintervalle sollten einzelnen Pathologien angepasst werden, da unterschiedliche Strukturen unterschiedliche Regenerationszeiten beanspruchen. Langzeiteffekte sollten noch zusätzlich dokumentiert werden. Es muss beachtet werden, dass die ausgewählten Techniken nicht dezidiert die Kausalität der Beschwerden der einzelnen ProbandInnen löst.Konklusion:Die Ergebnisse der Studie zeigen, dass eine osteopathische Behandlung an der HWS bei Schulterschmerzen wirksam ist. Abstract
Title:Influence of an osteopathic treatment on shoulder painBackground:Disorders of the shoulder are among the most common causes of pain in the musculoskeletal system of the body.Objective:This study examines the potential benefits of an osteopathic treatment of the neck in patients suffering from shoulder pain.Design:Randomized, controlled trial with 28 subjects distributed equally into a study and a control group. A combination of 4 types of treatment was applied twice to the neck of the study group. It received the first treatment at the beginning of the study; the second treatment 14 days later. The control group was treated with transcutaneous electric nerve stimulation (TENS) in the same time interval. The effects of the therapy were measured using three methods: the Visual Analog Scale (VAS), the Constant-Murley score (CMS), and the shortform 36 (SF36) health survey questionnaire.Results:Both the study and the control group reported improvements over the course of the treatment. In the study group, improvements manifested more quickly, after the first treatment. After the second treatment, both groups displayed similar therapeutic effects. The study group made overall greater improvements in in the subcategory "force” in the CMS.Discussion:The interval times of testing and therapy should adapt to the different pathologies because the different muscular-skeletal structures take varying amounts of times to heal. It is also important to notice the time dependent effect.Conclusio:Osteopathic treatments at the cervical spine are helpful in dysfunctions and pain of shoulders. To get more reliable results, it would be necessary to adapt some parameters, for example the dosage and the time intervals between the separate treatments, and recruiting more subjects.Keywords:cervical spine, shoulder pain, osteopathic treatment,constant murley scoreÂ
