Abstract
Hintergrund: Das Postcholezystektomiesyndrom
(PCS) tritt laut Literatur bei 14 bis 34 % der Patienten auf. Das Ziel dieser
Studie ist einerseits zu untersuchen, ob die Durchführung der Primärstudie
machbar ist, und andererseits, ob Osteopathie Auswirkungen auf den Schmerz und
die gesundheitsbezogene Lebensqualität beim PCS hat.
Methoden: randomisierte, kontrollierte Studie (n=14), zwei Gruppen: Versuchsgruppe
(VG), osteopathische Behandlung (n=8) und Kontrollgruppe (KG), physiotherapeutische
Behandlung (n=6). Der primäre Messparameter war Schmerz (NRS) und der sekundäre die
gesundheitsbezogene Lebensqualität (SF-12). Drei Messungen wurden durchgeführt ?
in der vierten Woche postoperativ (p.o.), in der achten Woche p.o. und in der
zwölften Woche p.o.
Ergebnisse: 100 % der VG gaben in der
achten Woche p.o. eine komplette Schmerzfreiheit an. Die positiven Ergebnisse
wurden beim Follow up erneut bestätigt. In der KG berichteten 100 % der
Probanden über keine Schmerzänderung zwischen der vierten und achten Woche p.o.,
jedoch in der zwölften Woche zeigten die Testergebnisse eine allgemeine
Verbesserung um einen Schmerzgrad. Die gesundheitsbezogene Lebensqualität
verbesserte sich in der VG verglichen mit der KG deutlicher. Die
Forschungsergebnisse zeigen zwischen den beiden Gruppen klare Unterschiede,
welche klinisch relevant sind.
Schlussfolgerung: Die Machbarkeit
der Studie gestaltete sich in Punkten der Probandenrekrutierung und Erreichung
der Mindestteilnehmeranzahl als problematisch. Anhand der Ergebnisse konnte
herausgefunden werden, dass durch die osteopathische Intervention ein Einfluss
auf das PCS bezogen auf den Schmerz und die gesundheitsbezogene Lebensqualität
stattfindet. Für Folgestudien ist eine größere Anzahl von Probanden
erforderlich. - Type

ABSTRACT
Background: According to literature data, post-cholecystectomy syndrome occurs in 14 to 34 percent post-operatively. The aim of this study is to investigate whether the study is feasible and whether osteopathy has an impact on the pain and health-related quality of life in post-cholecystectomy syndrome.Methods: randomized controlled trial (n=14), division into two groups ? intervention group (n=8) and control group (n=6). The primary measurement parameter was pain (NRS) and the secondary health-related quality of life (SF-12). The subjects were evaluated a total of three times ? in the fourth week postoperatively (p.o.), in the eighth week p.o. and in the twelfth week p.o.Results: 100 % of the intervention group reported complete freedom from pain in the eighth week p.o. The positive results were confirmed again at follow up. In the control group, 100 % of the subjects reported no change in pain between the fourth and eighth week p.o., but at the twelfth week the test results showed a general improvement by one degree of pain. The health-related quality of life improved more significantly in the intervention group compared to the control group. The research results show significant differences between the two groups, which are clinically relevant. Conclusion: The feasibility of the study turned out to be problematic in terms of subject recruitment and the achievement of the minimum number of participants. Based on the results it could be found that the osteopathic intervention has an influence on the post-cholecystectomy syndrome in relation to the pain and the health-related quality of life. Follow up studies require a larger number of subjects.
