- HintergrundKinder mit Lippen-Kiefer-Gaumenspalten haben häufiger pathologische Mittelohrbefunde als Kinder ohne Spaltdefekt und demnach stellen Mittelohrpathologien ein großes Problem in der Behandlung der Spaltkinder dar. Das Ziel dieser Pilotstudie war, eine erste Abschätzung eines möglichen Zusammenhangs zwischen Mittelohrpathologien und der Mobilität des Os temporale bei Säuglingen und Kleinkindern mit Spaltdefekt zu bekommen.MethodeIm Rahmen dieser Arbeit wurden Befunde des Mittelohres durch Hals-Nasen-Ohren ÄrztInnen erstellt und bei denselben Kindern die Mobilität des Os temporale auf beiden Seiten mittels Palpation durch eine Osteopathin getestet. Die 20 StudienteilnehmerInnen waren zwischen 3 und 24 Monate alt und hatten eine Form der Lippen-Kiefer-Gaumenspalte. Folglich wurden die beiden Ergebnisse gegenübergestellt und statistisch ausgewertet.ErgebnisseIn Hinsicht auf die primäre Fragestellung zeigte sich eine Übereinstimmung hinsichtlich pathologischer Befunde der HNO Untersuchung und einem blockierten Os temporale bzw. blander HNO Befunde und frei beweglichem Os temporale bei 17 Kindern. In Anbetracht der Tatsache, dass die Befunde jeweils pro Kind an zwei Ohren bzw. zwei Ossa temporalia erstellt wurden, ergibt sich eine Übereinstimmung bei 36 von 40 Ohren und demnach 90%. Der Kappa-Wert lag bei 0,794.SchlussfolgerungDie Ergebnisse zeigen, dass es einen möglichen Zusammenhang zwischen blockiertem Os temporale und Mittelohrpathologien bei Spaltkindern geben kann. Dennoch sei anzumerken, dass die vorliegende Arbeit auf Grund der geringen Fallzahl in ihrer Aussagekraft eingeschränkt ist und die Ergebnisse vorsichtig zu interpretieren sind. In jeder Hinsicht verstärkt dieses Pilotprojekt jedoch die Notwendigkeit weiterer Forschung auf diesem Gebiet, um klarere Aussagen und Empfehlungen für die Praxis abgeben zu können.

- BackgroundChildren with cleft lip and palate suffer from middle ear pathologies more often compared to children without clefts. Therefore, middle ear pathologies represent a serious problem in the treatment of children with cleft lip and palate. The objective of this pilot study was to assess a possible correlation between middle ear pathologies and the mobility of the temporal bone in children with cleft lip and palate.MethodFor this pilot study, 20 participants aged 3 to 24 months with any type of cleft lip and palate were included. Middle ear examinations were performed by an ear, nose and throat doctor and in the same children the mobility of the temporal bone was assessed by an osteopath. Results were compared and statistically evaluated.ResultsIn 17 out of 20 participants, a match was found between pathologic results of the ear, nose and throat doctor and a blocked temporal bone and on the other hand unremarkable results of the middle ear examination and a freely movable temporal bone. Taking into account the results of two ears and two temporal bones per child, there was a consistency in 36 of 40 ears corresponding to 90%. Cohen´s Kappa coefficient was calculated with 0.794.ConclusionThe present study reveals a consistency between middle ear pathologies and blocked temporal bones in children with cleft lip and palate in the majority of the examined participants. However, due to the low sample size our findings have to be interpreted with caution. Further studies covering this topic have to be performed in order to confirm our results and develop recommendations for the daily clinical work with children suffering fromcleft lip and palate.
