Einleitung:
Pregnancy-related
low back pain (prLBP) und Pregnancy-related
pelvic girdle pain (PPGP) zählen zu häufigen Beschwerden in der Schwangerschaft
und post partum. Es kann dabei zu Schmerzen und funktionellen
Beeinträchtigungen bei Aktivitäten des täglichen Lebens kommen. Die Studie
untersucht die Effektivität von Osteopathie bei prLBP und PPGP während der
Schwangerschaft und post partum. Des Weiteren wurden der therapeutische Zugang
der behandelnden OsteopathInnen genauer betrachtet und die diagnostizierten osteopathischen
Dysfunktionen dokumentiert.

Methodik:
Im
Zeitraum April bis August 2017 wurde eine systematische Literaturrecherche in medizinisch
relevanten Datenbanken und Zeitschriften vollzogen. Die inkludierten Studien
wurden mit der PEDro Skala auf ihre methodologische Qualität hin beurteilt.

Ergebnisse:
Von
eingangs 363 Studien verblieben 9 zur methodologischen Qualitätsbeurteilung. Alle
Studien wiesen eine niedrig bis moderate Qualität der Evidenz auf. Es zeigte
sich eine Tendenz einer klinisch relevanten Reduzierung der Schmerzen und einer
positiven Veränderung funktioneller körperlicher Einschränkungen im Alltag.
Zwei Studien arbeiteten nach einem vorgegeben Therapieprotokoll, bei allen
anderen war der therapeutische Zugang individuell befundorientiert gestaltet.
Es konnten Informationen zu den spezifischen osteopathischen Dysfunktionen gewonnen
werden.

Schlussfolgerung:
Das
Review weist auf eine Tendenz eines positiven Effekts von Osteopathie auf
Schmerzreduktion und Reduzierung funktioneller Einschränkungen bei Schwangeren
mit prLBP und PPGP und post partum hin. Es besteht ein Mangel an qualitativ
hochwertigen und großangelegten Studien. Ganzheitlich orientierte, randomisiert
kontrollierte Studien im osteopathischen Feld, mit einer klaren Dokumentation des
Befundes und der Behandlung, sind für die Zukunft wünschenswert.

ABSTRACT



Introduction: Pregnancy-related low back pain (prLBP) and Pregnancy-related
pelvic girdle pain (PPGP) are common complaints of pregnant women and often
remain postpartum. Besides pain, these pregnancy disorders can cause disabling
problems in everyday life. The
present study aims to examine the effectiveness of osteopathic treatment on
pain and functional status, to clarify the way of osteopathic approach and at
last to find out more about common osteopathic findings in prLBP and PPGP.

Method:
A systematic literature search was performed by the author from April to August
2017 to identify relevant articles in medical databases and journals. The evaluation
of the quality of the articles was executed using the PEDro scale.

Results:
After checking 286 studies on inclusion and exclusion criteria to fit into the
present review, 9 studies remained for PEDro rating. In 2 studies the
osteopathic approach was designed with a given protocol, the others were using
the black box model, which represents the individual osteopathic approach,
based on a clinical examination. Low to moderate evidence suggested a clinical
relevant decreasing effect on pain and increasing functional status in women
with prLBP and PPGP. Data of the osteopathic findings and approach could be
collected.

Conclusion: The present review submits a trend of clinical
relevant benefits of osteopathy for pregnant or postpartum women with prLBP or
PPGP. There is a lack of high quality studies and a detailed documentation
about the therapeutic approach in the osteopathic field. Larger high quality
randomized controlled trials with control groups and a straight defined
methodology are highly recommended.
