Hintergrund: Der
sogenannte Auskultationstest der A. Carotis ist, im Rahmen von Thrust an der
Halswirbelsäule, ein vorangehender Sicherheitstest. Ein positives Testergebnis
stellt eine Kontraindikation für eine Thrustbehandlung dar. Da die Bewertung
der Auskultation jedoch zum vorwiegenden Teil von der Erfahrung des Testers
abhängt (Gahl, Fischer, & Gebel, 2014) und
nur zum geringen Teil vom Stethoskop (Röhl, 1984), ist der Autor der Studie der
Meinung, dass Osteopathen mit physiotherapeutischem Hintergrund dieses
Testverfahren nicht aussagekräftig interpretieren können.

Es haben 32 Probanden an der Studie teilgenommen. Das Alter
reichte von 29 bis 73 Jahren. Ausschlusskriterium stellte ein vorhandenes
Tracheostoma dar. Die Einschlusskriterien waren eine unterschriebene
Einverständniserklärung.

Die Untersuchungen wurden von zwei Osteopathen mit
abgeschlossenem 5. Jahr an der WSO durchgeführt. Die Kontrolluntersuchung
mittels einem Ultraschallgerät wurde von einem Arzt gemacht.

Ziele: Ziel der Studie
war es aufzuzeigen ob die Osteopathen untereinander bzw. im Vergleich zum
Ultraschall zu denselben Ergebnissen gelangen.

Methode: Die Probanden
wurden von zwei Osteopathen im Halsbereich untersucht. Anschließend folgte eine
Ultraschalluntersuchung von einem Arzt. Die Probanden gingen von einer
Untersuchung zur nächsten.

Resultate: Es fand sich keine statistische Signifikanz
zwischen den Ergebnissen der Auskultation und den Ergebnissen des Ultraschalls.
Die Interraterreliabilität zeigte eine statistische Signifikanz auf.

Diskussion: Diskussionspunkte
stellen die Qualität der Stethoskope und die geringe Teilnehmerzahl dar.

Title: Validity and Inter-rater Reliability Study of

Study design: Inter-rater reliability and validity study

Background: In the context of thrust applied to the
cervical spine a previous safety test, the so-called auscultation test of the
carotid artery, is indicated. A positive test represents a contraindication for
a thrust. However, since the interpretation of the auscultation depends mainly
on the experience of the tester (Gahl, Fischer, & Gebel, 2014) and hardly on the kind of stethoscope that is
used (Röhl, 1984). The author of the study thinks
that osteopaths with a background in physical therapy are not able to
adequately interpret what they hear.

32 test persons
participated in the study. Their age ranged between 29 and 73 years. The
exclusion criterion was a present tracheostoma. The inclusion criterion was a
signed declaration of consent.

The examinations
were carried out by two osteopaths who already finished their 5th year
of osteopathic training at the Vienna School of Osteopathy. The control
examination was carried out by means of an ultrasound instrument by a medical
doctor.

Objective: To evaluate whether the osteopaths correlated
in their examination results with each other and with the result of the
ultrasound examination.

Method: The test persons proceeded from one
examination to the next.

Results: No
statistical significance could be identified between the results of the
auscultation and the results of the ultrasound examination. The inter-rater
reliability showed a statistical significance.

Discussion: Subjects of debate are the quality of the
stethoscopes and the small number of participants.
