dieses systematischen Reviews ist die Effektivät der Osteopathie auf den
Cortisolspiegel anhand der bestehenden wissenschaftlichen Evidenz zu
untersuchen.

Methodik: Für diese Übersichtsarbeit wurden die Datenbanken PubMed, Cochrane
Library, Ostmed-Dr, Research Gate, Osteopathic Research Web sowie im „Journal
of the American Osteopathic Association“ und im „International Journal of
Osteopathic Medicine“ nach geeigneten Studien anhand spezifischer Ein- und
Ausschlusskriterien gesucht. Die Literaturrecherche fand im Zeitraum vom 15.
Februar bis 5. Juni 2020 statt. Für die Qualitätsanalyse der ausgewählten
Studien wurden die Downs und Black Checkliste verwendet. Die Übersichtsarbeit wurde
in Anlehnung an das PRISMA Statement verfasst.

Resultate: 15 Studien wurden in das systematische
Review für die weitere Analyse aufgenommen. Bei neun von 15 Studien zeigten die
Messungen keine signifikante Änderung des Cortisols. Eine von zwei Studien mit
osteopathischen Behandlungen und eine von zwei Studien mit Techniken aus der
craniosacralen Osteopathie zeigten eine signifikante Reduktion von Cortisol. High
Velocity Low Amplitute Thrust (HVLAT) Techniken bzw. Manipulation der
Wirbelsäule konnten bei fünf von neun Studien keine signifikanten Veränderungen
des Cortisolspiegels erzielen. Einzelne Techniken wie die
Myofascial-Release-Technik oder die Rib-Raising-Technik konnten in den Studien keine
signifikanten Unterschiede der Cortisolwerte direkt nach der Intervention bewirken.

Conclusion: Durch die geringe Probandenzahl in beinahe
allen Studien und vorhandene Mängel in der Studiendurchführung bleibt unklar,
ob die Osteopathie Cortisol wesentlich beeinflussen kann. Die ausbleibenden
Effekte und die teils widersprüchlichen Ergebnisse bei der Mehrzahl an Studien könnten
auf die Limitationen insbesondere auf die Messmethodik und die mangelhafte
Berücksichtigung der Störfaktoren zurückzuführen sein.

Abstract
en Objective: The objective of this systematic review was to
evaluate the current scientific evidence concerning the influence of osteopathy
on cortisol levels.

Method: To
find relevant Articles for this systematic review, an electronic literature search
in the databases of PubMed, Cochrane
Library, Ostmed-Dr, Research Gate, Osteopathic Research Web as well as in the
„Journal of the American Osteopathic Association“ and the „International
Journal of Osteopathic Medicine“, was conducted. The selection was based on
specific inclusion and exclusion criteria. The literature search was performed between
the 15th of February to the 5th of June 2020. The systematic review was done
according to the PRISMA Statement and the quality of the selected studies was
evaluated using the Downs and Black Checklist.

Results: 15 studies were included in this review for
further assessment. Nine of 15 articles showed no significant change in
cortisol levels both within- and between- groups. One of two studies using an
osteopathic manipulative treatment intervention and one of two studies using
craniosacral techniques found a significant reduction of cortisol. Five of nine
studies did not find significant differences in the proportion of cortisol
after High Velocity Low Amplitude Thrusts of the spine or spinal manipulations.
A rib raising and a myofascial release technique could not alter cortisol
levels significantly immediately after the intervention.

Conclusion: Due to small sample sizes and low quality in
almost all studies, it remains unclear, if osteopathic manipulative treatment
has a sufficient effect on cortisol. Limitations in cortisol collection methods
and the inadequate control of confounding factors are possible reasons for the
missing effects and the mixed evidence in the majority of the studies.
