- Title: A critical review of current theories surrounding Chronic Fatigue Syndrome aetiology and the implications of these on osteopathic management of patients with the condition. Aim: The aim of this study is to review the latest relevant published studies, reviews and expert opinion articles to better understand the aetiology and pathophysiology of CFS. Moreover the researcher also analysed selected osteopathic (and other alternative treatments) papers to understand how they could benefit CFS patients and to evaluate the claims of these therapeutic interventions. Method: Systematic literature searches for randomised controlled trials from 2003 to 2013 using methodological quality guidelines. Keywords: chronic fatigue syndrome, manual therapy, osteopathy Results: Overall the research was of variable quality. After scoring, the researcher will use forty-one articles: twenty-eight non-randomised clinical trials, four randomised clinical trials and nine systemic reviews. Conclusion: In conclusion, CFS is still a misunderstood, disabling condition of unknown specific aetiology. The main hypotheses include altered central nervous system functions resulting from an abnormal immune response against a common antigen; a neuroendocrine disturbance; cognitive impairment caused by response to infection or other stimuli, such as emotional or physical stress, in sentient people. Concerning CFS management it seems like manual therapy and osteopathy are beneficial, as they improve CFS symptomology. Combined with CBT (cognitive behavioural therapy) patients report decrease in fatigue, alleviation of pain and improvement of cognitive functions.
